Gatka Competition Liability Waiver and Release Form
I, the undersigned participant, or parent/guardian of the participant named below (hereinafter referred to as "I" or
"Participant"), voluntarily participate in the Gatka competition (the "Event") organized by Art, DHAN BABA BUDDHA SAHIB JI GATKA AKHADA,DALPANTH WINNIPEG AND SIKH SOCIETY PF MANITOBA
collectively referred to as the "Organizers").
Participant's Certification:
I certify that I am physically fit and have adequately prepared myself for participation in this Gatka competition. I
understand and acknowledge that there are potential risks associated with Gatka, including but not limited to, injuries
caused by the use of sharp weapons, physical contact during fights, and other related activities. I confirm that I have not
been advised against participation by a qualified medical professional and that there are no health-related reasons or
problems that prevent my participation.
In consideration of being permitted to participate in the Event, I agree to the following terms and conditions:
1. Assumption of Risk: I acknowledge that participation in the Event involves inherent risks, including but not limited
to bodily injury, disability, death, property damage, and property theft, which may result from negligence, gross
negligence, fault, or misconduct of others, including participants, spectators, organizers, volunteers, and any third
parties.
2. Waiver and Release: I hereby waive, release, and discharge all claims, demands, actions, or causes of action against
DHAN BABA BUDDHA SAHIB JI GATKA AKHADA,DALPANTH WINNIPEG AND SIKH SOCIETY PF MANITOBA
directors, officers, employees, volunteers, representatives, agents, sponsors, and affiliates (collectively, the
"Released Parties") from any and all liability for any loss, damage, injury, disability, death, property damage, or
property theft arising out of or related to my participation in the Event, regardless of whether such liability arises
from negligence, gross negligence, fault, misconduct or otherwise.
3. Indemnify, hold harmless, and promise not to sue: I agree to indemnify, defend, and hold harmless the Released
Parties from any and all liabilities, claims, actions, damages, costs, or expenses, including attorney fees, arising from
or related to my participation in the Event, including any claims of bodily injury, disability, death, property damage,
property theft, negligence, gross negligence, fault, misconduct, or liability without fault, whether caused by the
Released Parties or others.
4. No Admission of Fault: This Agreement shall not be construed as an admission of fault, liability, or wrongdoing on
the part of the Released Parties. It is a voluntary acknowledgment of risks and a waiver of certain legal rights.
5. Comprehensive Release: This Release and Indemnification Agreement shall be effective and binding upon me, my
heirs, executors, administrators, legal representatives, successors, and assigns.
6. No Warranty: I acknowledge that the Organizers make no representations or warranties regarding the safety,
suitability, or condition of the premises, equipment, or activities involved in the Event.
7. Medical Treatment Authorization: In the event of any injury, accident, or illness during the Event, I authorize the
Released Parties to obtain or provide necessary medical treatment deemed advisable.
8. Photographic and Video Release: I consent to the use of my name, likeness, voice, and image in photographs,
videos, or recordings for promotional or educational purposes related to the Event, without compensation or
further consent.
9. Compliance with Rules: I agree to comply with all rules, regulations, instructions, and decisions of the Event
organizers and officials.
Acknowledgment of Understanding:
I have read and fully understand the terms and conditions of this Liability Release Form. I acknowledge that this form
constitutes a release of liability and a contract between myself and the Released Parties. I voluntarily sign this form as my
own free act and agree to be bound by its terms.
